Eligibility

The eligibility requirements for iCircle depend on whether you wish to enroll in iCircle Care:

  • iCircle Care Eligibility Guidelines:
    iCircle Care is for those who need long-term care for more than 120 days due to chronic illness and/or a disability.1 Additionally, you must meet the following requirements:
    • 18 years of age or older
    • Resident of 22-County Service area (listed below)
    • Determined eligible for Medicaid by LDSS or entity designated by the State
    • Determined eligible for Managed Long Term Care, using an eligibility assessment tool designated by the NYSDOH, if applicable
    • Capable, at time of enrollment of returning to or remaining in his/her home and community without jeopardy to his/ her health and safety, based upon criteria provided by NYS DOH.
    • Require at least one of the following services for more than 120 days from effective enrollment date:
      • Nursing Services in the Home
      • Therapies in the Home
      • Home Health Aide Services
      • Personal Care Services in the Home
      • Adult Day Health Care
      • Private Duty Nursing
      • Consumer Directed Personal Assistance Services

Enrollment in iCircle Care is voluntary1 and members can disenroll at any time, for any reason.

1 Individuals who reside in Monroe or Onondaga County and meet the eligibility criteria for MLTC are required to enroll in an MLTC Plan.

How to Enroll in iCircle Care

  1. Call 1-844-iCircle (1-844-424-7253). You will speak with a Member Services Representative who will provide you with an overview of iCircle Care and help determine whether you meet the eligibility criteria.
  2. iCircle will need to determine whether you are a current recipient of Medicaid. If not, we can arrange for a Medicaid Specialist to contact and assist you in completing a Medicaid Application.
  3. Once you decide to enroll, an Enrollment Nurse will schedule an in-home visit to complete a comprehensive assessment of your health and safety, as well as your home and environmental surroundings.
  4. From these assessments, a personalized plan of care will be generated.
