iCircle Care is a NYSDOH Certified Medicaid Managed LongTerm Care Plan that assists people who are chronically ill or disabled and require health and long-term care services through administration of home care, personal care, social supports, transportation, skilled nursing facility, and/or other services. iCircle Care coordinates and pays for all medically necessary services in the benefit package for their members. Interested persons who meet the following criteria are encouraged to complete a Pre-Enrollment Referral form to receive information on how to enroll in iCircle Care:

• Be eighteen (18) years of age or older
• Reside in one of the following service counties: Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Cortland, Delaware, Erie, Genesee, Herkimer, Livingston, Madison, Monroe, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Otsego, Schuyler, Seneca, Steuben, Tioga, Tompkins, Wayne, Wyoming, or Yates
• Has Medicaid or qualifies for Medicaid.
• Be capable of staying in your home without jeopardy to your health and safety
• Require 1 of the qualifying Community-Based Long-Term Care Services for more than 120 days from the effective date of enrollment.
